Success Story and Testimonial Release

Last updated: September 11, 2026

Thank you for sharing your story. Your words help someone else take a first step, and we do not take that lightly.

This release explains exactly what you are giving us permission to do with what you send, and what stays yours. Please read it before you check the box.

1. Who This Is Between

This release is between you and The Vitality Project, LLC, a Utah limited liability company, and its founder Elizabeth Faye, its employees, contractors, agents, and the partners and platforms it authorizes to publish on its behalf. In this document, "we," "us," and "our" mean The Vitality Project, LLC. "You" and "your" mean the person submitting.

2. Definitions

  1. Submission. Everything you send us through the Share Your Story form or any other story or testimonial channel: your written story, your title, your captions, your video, your audio, your photo, your Instagram handle, your name and role, and anything else you include.
  2. Materials. Anything we create that includes or is based on your Submission: social posts, website pages, app screens, emails, printed pieces, slides, videos, podcast segments, press materials, sales pages, and advertising.
  3. Likeness. Your name, your business or salon name, your city and state or country, your professional title, your photograph, your image, your voice, your video appearance, your social handle, and anything else that identifies you.

3. What You Are Giving Us

You grant us a non exclusive, worldwide, royalty free, fully paid license to use your Submission and your Likeness as described below.

Non exclusive means you keep every right to your own story. You can tell it anywhere, to anyone, as often as you like, including to other companies. Nothing here stops you.

You keep ownership. This is a license to use, not a transfer. Your story stays yours. Your photo, your video, and your voice stay yours. We are not buying them, and we are not claiming copyright in them.

3.1 What we may do

  1. Publish your Submission, in whole or in part.
  2. Edit for length, grammar, spelling, and clarity, without changing what you meant. We will not put words in your mouth, change your meaning, reverse your point, or make your experience sound bigger than you described it.
  3. Excerpt a portion, such as a single sentence pulled as a quote.
  4. Combine your Submission with other members' stories, with photos, with music, with graphics, and with our own copy.
  5. Format and adapt it for different sizes, screens, and channels, including captions, subtitles, cropping, and color correction.
  6. Translate it into other languages.
  7. Attribute it to you, using the name, role, and location you gave us, and tag your social handle if you gave us one.
  8. Archive it, and keep copies of what we have already published.

3.2 Where we may use it

  • The Vitality Project website and the member app
  • Our community spaces and member communications
  • Email to our list
  • Social media, including Instagram, TikTok, YouTube, Facebook, LinkedIn, and Pinterest
  • Podcasts, press, editorial features, and interviews
  • Live presentations, keynotes, trainings, retreats, and events
  • Sales pages, brochures, media kits, and other marketing materials
  • Paid advertising on any channel. See Section 4.
  • Worldwide, in any language, in any medium now known or later developed

3.3 What we will not do

  1. We will not change what you meant.
  2. We will not use your Submission to endorse a product, brand, or company other than The Vitality Project without asking you separately.
  3. We will not sell or license your Submission to another company for their own marketing.
  4. We will not use your Submission to train an AI model, and we will not permit anyone else to.
  5. We will not publish a health detail you told us to leave out. See Section 7.

4. Paid Advertising

Paid advertising is included. The uses in Section 3.2 expressly include paid advertising on any channel: sponsored posts, boosted content, and paid placements on social platforms, search, streaming, podcasts, display, print, and broadcast, worldwide.

When your story runs as an ad:

  1. The platform may show it to people outside our community, including people who have never heard of us.
  2. Platforms keep ads in public ad libraries. Some of those libraries stay visible after a campaign ends, and we do not control them.
  3. We still credit you the way you asked us to, and everything in Section 3.3 still applies. We will not put your story behind a claim we cannot support, and we will not use it to sell someone else's product.

5. How Long This Lasts

This permission is perpetual and irrevocable. It lasts indefinitely and it cannot be withdrawn. Once your story is published, we do not have to remove it.

You can still ask. Email support@jointhevitalityproject.com at any time and tell us you would like us to stop using your story in future materials. We read every request, and we may agree at our discretion where there is an important reason, for example a safety concern, a privacy concern, or a family situation. We are not obliged to agree, and this is a decision we make case by case.

Even when we do agree, some things cannot change. Anything already printed, already running as an advertisement, embedded in a video, built into a presentation or a deck, already sent as an email, already published as a podcast episode or a press feature, reposted by someone else, or otherwise not reasonably changeable stays exactly as it is. We cannot recall it, and we are not responsible for material other people hold or republish.

We keep an archive copy of what we published and of your release, so we have a record of what was authorized. It is not used for anything else.

6. What You Are Confirming

By submitting, you confirm that:

  1. It is true. Your story is accurate and describes your actual experience, in your own words.
  2. It is yours. It is your own experience, and you wrote, recorded, or filmed what you sent, or you have the right to send it.
  3. Other people in it have agreed. If your Submission names, shows, or is about anyone else, including a team member, a client, a student, a partner, or a family member, you have their permission to share it with us and for us to publish it. If a child appears, you have their parent's or guardian's permission.
  4. You have not shared anyone's confidential information. No client records, no employee records, no health information about another person, no financial details of another business, and nothing covered by an agreement you have signed with someone else.
  5. Nothing in it infringes anyone's rights. Any music, photography, artwork, logos, or writing by someone else is either yours to use or properly licensed, and you have the right to include it.
  6. You are not making claims we cannot support. Please describe what happened for you. Please do not say our programs cure, treat, prevent, or diagnose any medical or mental health condition, because that is not what they do and we cannot publish it.
  7. You are at least 18. By submitting, you confirm that you are 18 or older. We do not accept stories under this release from anyone under 18.
  8. You have the authority to sign. If you are sharing on behalf of a salon, school, or business, you are authorized to do so.

7. Health and Personal Details Are Yours to Control

Many of the best stories touch something personal: burnout, anxiety, grief, recovery, a diagnosis, a hard season. You never have to include any of that.

  1. Sharing a health detail is entirely optional. Your story is welcome without it.
  2. Anything you include, you are choosing to make public. Once it is published, it is public, and it can be seen by your clients, your team, your employer, your family, and anyone else.
  3. You can tell us what to leave out. Use the notes field, or email us. We will honor it.
  4. You can ask to be less identifiable. First name only, no photo, no city, no salon name, or a pseudonym. Just ask.
  5. We will check with you before we publish. We read every story and we reach out before anything is shared, so you get to see how your words will appear.

8. Age

You must be 18 or older to submit a story. By submitting, you confirm that you are.

9. Compensation and Disclosure

  1. We do not pay or otherwise compensate anyone for a story, testimonial, or review. You are not being paid for this, and you are not receiving a discount, free or extended membership, a product, a gift, event access, travel, or anything else in exchange for it. You are sharing because you want to.
  2. You are not owed royalties, residuals, or a share of anything we earn from Materials that include your Submission.
  3. If you are an affiliate, or you otherwise receive commissions from us, tell us. That relationship must be disclosed when your story is used, and we will note it wherever your story appears, as the FTC requires. This applies even though you received nothing for the story itself. The same goes for a partner, a brand ambassador, our employee, or our contractor.
  4. When you share your own story on your own channels, and you have any material connection to us, including an affiliate relationship, please disclose it clearly, in the post itself, not only in a bio or behind a "more" link.
  5. Your results are yours. Wherever we publish your story, we say that individual results vary and that your experience may not be typical.

10. We Might Not Publish It

We are not obligated to use your Submission, and choosing not to is not a judgment of you or your story. We read every one. We may also decide later to stop using something we have used before, without notice and for any reason. Nothing in this release requires us to publish, to keep publishing, or to publish in any particular way, place, or timeframe.

11. Approval and Waiver

You will see how your story is used before it is published, because we reach out before we share. Beyond that, you waive any right to inspect or approve each individual Material, each edit, each caption, each crop, and each placement, and you waive any claim arising from the way Materials are composed, edited, or distributed, as long as we stay within Section 3 and do not change what you meant.

12. Release of Claims

To the fullest extent permitted by law, you release The Vitality Project, LLC, Elizabeth Faye, and their employees, contractors, agents, licensees, and authorized partners from any claim arising out of the use of your Submission and Likeness as permitted by this release, including claims for defamation, invasion of privacy, false light, right of publicity, misappropriation, emotional distress, and copyright infringement.

This release does not cover any use outside what Section 3 permits, and it does not limit any right that cannot be waived under applicable law.

13. Everything Else

  1. The whole agreement. This release, together with our Terms of Service, is the entire agreement about your Submission, and it replaces any earlier conversation or understanding about it.
  2. Governing law and disputes. This release is governed by the law of the State of Utah, and any dispute is resolved under the dispute resolution, arbitration, venue, and limitation provisions of our Terms of Service, which are incorporated here.
  3. Electronic signature. Checking the box and submitting the form is your electronic signature, and has the same effect as signing on paper.
  4. If part of this is unenforceable, the rest still applies.
  5. Assignment. We may assign this release to a successor if our business is sold or reorganized. The same limits continue to apply.
  6. Survival. Sections 3, 5, 6, 9, 11, 12, and 13 survive.
  7. Changes. We may update this release for future submissions. The version you accepted governs your Submission, and we keep a copy of it.

Questions, changes, corrections, and requests to stop future use all go to the same place, and a person reads them. You may ask us to stop future use and we will consider every request, but because the permission in Section 3 is irrevocable, approval is not guaranteed.
